Enshittification! J’accuse!

Okay, this is a first world rant. Part of the first world blues I’m always singing.

My recurring prescription drugs are supplied through Express Scripts. And it works great. Except when it doesn’t. It didn’t this week.

I received a message from them to give them authorization to start an autofill on one of my prescriptions. I logged in and did as required. Another message came in: you have one item in your cart. Please complete your purchase.

WTAF?

I didn’t ‘complete the purchase’. I gave them feedback. Asked them to call.

Which they did. It was Kelly. She explained why she was calling in a chipper, professional voice edged with a little nervous quiver. I explained why I was annoyed. How I felt the system was telling me to do two different things. She then began explaining to me why my interpretation wasn’t correct. Nothing she was saying aligned with the messages or my experience. Reaching the point of irritation and recognition that nada was getting changed, I thanked Kelly and prepared to hang up.

“Well, do you want me to take care of getting the autofill restarted?” she asked.

Well, I thought I’d done that when I logged in and clicked on a button to start autofill. “Yes, please,” I answered. Kelly talked through the process of what had happened, what she was doing, and…

Her tone faltered. I sensed that she saw exactly what I meant in my complaint. Then, she finally said in a low voice, “Sometimes this system doesn’t make sense.”

Vindication!

I smiled.

“Is there anything else I can help you with?” Kelly asked.

My smiled stayed. “No, Kelly. You have a great day and a great week.”

“Why, thank you. You, too.”

With that, we said our goodbyes and I basked in my tiny, tiny victory.

Friday’s Wandering Thoughts

I had a medical appointment the other day. Met with a PA about my upcoming surgery. We had a good time with the young guy. My wife had helped host a birthday party for her Y exercise class instructor and brought home some goodies, so we were on a sugar high, cracking jokes at him. He, for his part, confessed that he wanted another cup of coffee and shared a story about how he’d once unwittingly consumed the ‘half caf’ that his parents brew.

Part of the directions to me for my appointment was to bring all my medications.

I ignored that directive. My PCP is with Asante; my surgeon is independent but working with me through Providence. Both use Mychart to track me and communicate. My medical prescriptions are in those records.

I’ll tell you, I like Mychart. I go in there whenever I want to check on my history or look at what’s upcoming. It’s a significant improvement on filing a billion pieces of paperwork like we used to do in the military.

Number two with not taking my meds with me, I’d filled out a paper questionnaire at my first appointment. That’s what folks call a ‘hard copy’. I was required to list my medications on it.

I figured my meds were pretty covered. If their systems were having trouble tracking them, we have much larger problems, Hal.

Of course, my med list contains two items: Flomax and Amlodipine. Many men over fifty are on Flomax for prostate gland issues. That includes me. People experiencing hypertension are often prescribed Amlodipine, and I fall in that Venn diagram.

I know of patients who have a complex array of prescriptions. Like Mom. Even after helping her sort her medicines, pain killers, and aids several times, I don’t know how many she has. I’d guess over twenty. They help with her pain, breathing, sleeping, bowel movements, lungs, heart, digestion, blood circulation, side effects of the drugs, and side effects of the side effects of the drugs. She’s in network but it’s a couple networks.

If you’re seriously developing us bots and AI, I think a smart app to help track drugs for people and the healthcare industry needs a hand.

I suspect this medication business is going to get increasingly complex. We’ll need whatever help we can to manage it. I know Mom would certainly appreciate a bot that tracks her pills and tells her when to take what. Given the potential for mixing drugs that don’t get along, I’d like that for her, too.

One thing about my appointment the other day that I noticed was that my PA never brought up my information on the terminal in the examining room, and he barely glanced at the stuff I’d filled out. Nope, instead, he had a small fan of paperwork that he consulted.

The change from paper to computer is underway but it’s gonna be a long haul.

The Glasses

I broke down. It was time. I knew it – I’ve known it for years. I needed new glasses.

The issues aren’t costs or worries about my eyes. I dislike taking time out of the day to go somewhere, wait for a person to see me, etc. I’m an impatient person who tries protecting my writing time and routines. I’m like a grizzly mama about those things. Writing time and routines are my cubs, to beat that simile to a final death. This is due to a lifetime of giving and serving others, what people call ‘employment’. I sacrificed my writing dream for others’ worries and concerns. This is now my time.

COVID did play a role. So did changing insurance coverage. I no longer had any vision plan. That went away when I left IBM in 2015. Putting a year to it and doing the math is a thump on the head; that much time has already passed. It is not conceivable. Seems like just yesterday that I was dragging my ass out of bed before the sunrise and logging in, calling in, and doing all the other things to ensure I was electronically tied to the company. My work for them had been remote for the final eight years. The final three, visits to company facilities were cut to nil due to slashes in travel funding.

A new optometrist was required, too, as the other one had disappeared. My wife’s friends raved about the one at Costco. I was skeptical but they said she was great. “Make sure you go to her, though, and not her father.” Okay. An appointment was made, and they were right. She was good, as far as I know. $69 for a new prescription. In and out in twenty minutes. I liked those elements.

“Well,” she said. “You have very healthy eyes and your vision has improved. You’re probably having problems with your old glasses, aren’t you?”

Yup.

“They’re way too strong for you.”

Well, that was great news. I told my wife. She seemed stunned. “Your vision is getting better?”

Yup.

She turned away, muttering something about fairness. I didn’t press. I had some idea of her comment.

I found and ordered new prescription sunglasses from Costco. Didn’t really need normal glasses, I felt. Don’t use them often. My wife muttered something else.

The glasses would cost $129, I was told after I’d gone through the process. Whoa, wait. “What about protective lenses and the reflective coating, and all that?”

“It’s all included.”

I can see so much better now with my new sunglasses, and they’re much more stylish. They’re relatively large, making my big head look a little smaller.

We all need whatever edge we can find.

Saturday’s Theme Music

Today’s magic numbers in Ashland are… (Drumroll please.) 5:46 AM and 8:43 PM. These are the times when the Earth’s natural movement brings that sun into our area and takes it back away.

Cool air dominates today. We did hit 90 degrees F yesterday. Today it looks like high seventies/low eighties for us today.

The music comes from exchanges on someone else’s blog site. Medication was the topic, and the warning, “Don’t take whatever drug if you’re allergic to it.” Nice disclaimer, trying to shift blame from the medical profession over to the patient. “We told him not to take it if he’s allergic to it.” Meanwhile, most meds come with disclaimers about all the stuff they might do to you while they’re fixing you. You’ve probably heard/seen them, so I won’t repeat them. Naturally, they have drugs to deal with the side effects of your drugs. Then the drugs you take to fix those side effects have their own side effects, for which more drugs are prescribed. Like a pyramid scheme, isn’t it?

That took me to thinking about drug and pills and a song about the same from 2005, “Save Me” by Shinedown.

We often ask ourselves, “How did I get here? What went wrong?” Sometimes you didn’t do anything wrong. You just lived and shit happened. Genetics asserted themselves. You got trapped in another’s mess. A bullet goes astray, a car misses a turn, a good intention goes awry, a politician lies. Hell, the way messes are built and multiplied, it’s easy enough in this modern existence, innit?

Med Frustrations

Okay, gotta vent. This is one of those first world blues rants, the kind that deal with technology, systems, and customer service taken for granted that ends up failing and pissing me off.

I’m on two medical prescriptions these days. One is for my enlarged prostate and was prescribed for me when I experienced problems in peckerville in 2019. It’s all benign and is working well enough now.

The other prescription is for high-blood pressure. That was discovered as a result of my peckerville issues. I’d been borderline high pressure throughout my life but it was suddenly over two hundred thirty.

Changes were made in diet and exercise, and the prescription, Amlodipine, begun. I was always getting thirty day prescriptions. I shifted that to ninety day for convenience, and then, concurrent with COVID-19, I started using a mail service to refill my prescriptions back in February or so.

This was being done through Express Scripts. I set my meds up for auto-refill. All was going well. I was satisfied.

But, last week, I noticed I was down to nine Amlodipine pills. I hadn’t heard from Express Scripts. That surprised me. I went into my email and did a search to verify that I hadn’t received something from them. Nope. I logged into my account.

Finding the Amlodipine prescription, I noted that it wouldn’t be automatically refilled until 10/23. Well, that was too late. I put it in the cart and ordered it. Done and done. Went to my email. A confirmation email had been received. Wonderful. The system was working.

But…

Everyone expected the but. But what happened, I’m sure people are wondering, to set off the rant?

But, I didn’t hear anything else.

Days passed. I logged back into my online Express Scripts account after not receiving further emails. I checked my recent orders and shipments. Why, there are no recent orders and shipments. I searched via the order number they provided me. That order wasn’t found. I searched via the invoice number provided. Nothing.

WTH? I’d copied it from the email and pasted it in.

The email also had a ‘click here for order status’ button. I clicked it.

It took me to the login page, where I went through the same thing as before.

WTF?

I did this several times, re-reading the email and tracing steps, trying to understand what went wrong. I couldn’t. I’m sure this wasn’t good for my blood pressure.

I reached out to them via an email and explained my issue.

That was Wednesday evening. I heard from them today, Friday. One was an email. Call us. Two was a phone call. ANONYMOUS.

I didn’t answer that call; I don’t talk to ANONYMOUS in this day of scams where everyone and their dog is trying to con me, asking for donations, or pleading for political contributions. Their message: call us.

I called them. They claimed that the order put in was for my other medication. That was ordered the 18th. It’d been canceled because it was an overfill. By the way, my Amlodipine isn’t on auto-refill. Do I want to put it on auto-refill?

Seriously.

I don’t think this exchange did my blood pressure any good.

My response: how did my Amlodipine go from auto-refill to non-auto if it wasn’t the one that I ordered the other day?

Two, if I ordered a prescription, whether it was overfill or not, why didn’t I receive an email notifying me that it’d been canceled? Didn’t they think there was a reason I was ordering it? Isn’t it just good customer service to notify a customer when an order is canceled?

Three, if I ordered it on the 18th, why did I have an email from them that I received on the 17th telling me that they were working the order?

Four, if the Amlodipine wasn’t on auto-refill and they don’t show me or anyone else changing it, how was the order previously automatically refilled? Was that just magic?

They didn’t have answers for any of these things.

Perhaps I did push a wrong button on the 17th when I was processing online. Given their system, I find it doubtful. The product must be selected. Then, it’s right there in front of my face. Perhaps I had a brain fart and shifted from thinking Amlodipine to the other one, Tamsulosin. I’m not infallible.

And, yeah, given time differences of one or two hours, I suppose I can accept the idea that the order I put in at ten PM on the 17th showed up as being received on the 18th. That doesn’t explain the rest, especially the lack of an email telling me that the order had been canceled.

And that sucks.

Fortunately, I have an excellent local pharmacy. I shifted from them mostly because of the whole COVID-19 thing. Getting my meds through the mail with auto-refills took a few things off my place, thereby reducing stress, and eliminated the need to leave the house to get refills.

I called that pharmacy today for a short term refill. They were understanding. They would need to call my doc for the prescription, but no problem, they would do that. They’re so nice, I feel bad about abandoning them.

Then I called my doc’s office to provide a heads-up that they would receive a request, and why.

Well, the rant is done. I don’t feel any better after ranting. Too many loose ends. It’s gonna take a while to get over it. Meanwhile, my trust in Express Scripts has dropped significantly. There are just too many open questions about what happened for me to have complete trust. It’ll take time, probably years, for them to earn that. That’s what happens when a trust is breached.
